The purpose of this document is to summarize a wide range of assessment activities conducted by Stearns County Public Health, St. Cloud State University, and the Minnesota Institute of Public Health in order to better understand community needs, interests, readiness, and resources relevant to the State Health Improvement Program (SHIP) planning process. Data from other data sets including the Minnesota Student Survey and surveys conducted by the University of Minnesota on a number of higher education campuses in Minnesota were also reviewed and considered. This summary will provide a description of the assessment activities considered by the State Health Improvement Program (SHIP) Community Leadership Team (CLT) and is organized by the interventions selected by CLT the for implementation in selected communities in Stearns County.
